Our Mission

Hearts of Hope LLC’s mission is to assist individuals served, to be actively involved in their community, families, relationships, employment, and choices made in their daily living. We will ensure choices are person-centered, positive and will help the individual achieve their set goals.

Hearts of Hope LLC’s vision is for every individual with Intellectual/Developmental Disabilities and their families to be actively involved in their communities, where the individual feels safe, confident, and willing to try new things as they explore their abilities over their disabilities.

About Us

Hearts of Hope LLC, was founded by Co-Owners Jamie A. Ortega and Debbie C. Gonzalez, on June 4, 2021. We are committed in providing optimal care to individuals under the DD waiver, while keeping emphasis on person centered services, to achieve their quality outcomes, to have choices, live meaningful lives, and to engage in meaningful relationships in their community of choice. To offer premier opportunities to the individuals in becoming independent in our communities, supporting individual choices, and meeting their personal goals.

Our Services

Family Living:

This service is 24 hours living support provided to eligible individuals with developmental disabilities in their homes or in the residence of the direct service provider. Family Living Services are provided using non-shift staffing model in which the individual is supported as part of a family unit. The Family Living Services direct service provider shall not be the spouse of the individual served.

Supported Living Services:

This service provides in a single residence up to 4 or fewer individuals. The individual (s) in Supportive Living requires up to 24 hours of staffing per day.

Customized In-Home Supports:

This service provides individuals with access to participate in activities and functions of the community life.


This service allows the primary caregiver to take a break from care giving responsibilities while maintaining adequate supervision and support to the individual during the absence of the primary caregiver.

Customized Community Supports (CCS)/Individual Group (CCSIG):

CCS are based upon the preferences and choices of each person and designed to measure progress toward Desired Outcomes specified in the ISP. Activities include adaptive skill development, adult educational supports, citizenship skills, communication, social skills, self-advocacy, informed choice, community integration, and relationship building.

Adult Nursing:

Adult Nursing Services (ANS) are designed to meet a variety of health conditions experienced by adults receiving services on the DD Waiver Program. These services are intended to support the highest practicable level of health, functioning and independence for persons, age 21 years and older who:

1. reside in a Family Living setting.

2. receive Customized In-Home Supports.

3. require ANS but who do not receive Living Supports.

4. require ANS during participation in CCS-I or CCS-Small Group and/or CIE. ANS are also available for young adults, age 18 through 20, who: reside in Family Living and are at aspiration risk and who are required to have ARM supports.
